A+ Student Agreement

The A+ Student Agreement must be completed in order to acknowledge your participation in the program and to receive A+ tutoring opportunities.

A+ Timesheet

A timesheet must be submitted in order to count any volunteer and/or tutoring hours towards your A+ 50 hours.

A+ Attendance Appeal Form

This form must be completed by the student if their 4-year average attendance has dropped below 95% attendance.

A+ Discipline Appeal Form

This form must be completed by the student if they have been removed from the A+ Scholarship Program due to citizenship and/or behavior.
